Results from recent Linkedin Poll regarding which Incident Investigation/RCA Methodologies are being utilised:

What I found interesting is:

• COMET is the only new methodology developed (developed in the last 10 years)
• 5 Whys dominates as a preferred methodology
• Many indicated that they utilised multiple methodologies
• There are a lot of methodologies I didn’t know existed

I must also disclose that PSG (my company) is a COMET reseller and training provider.  I taught Taproot for 13 years but find COMET much better!
